10 mars 2025 (Oakville, ON)<\/b> \u2013 The CAN Health Network is proud to partner with Evidence at the Point-of-Care (EAPOC), a Canadian company that develops point-of-care chronic disease computerized decision support systems. EAPOC\u2019s flagship product, the Electronic Asthma Management System (eAMS)<\/a> provides personalized asthma guidance for both patients and health care providers.<\/p>\n

Malgr\u00e9 la disponibilit\u00e9 de traitements efficaces, environ 90 % des Canadiens asthmatiques ne parviennent pas \u00e0 bien contr\u00f4ler leur maladie, ce qui entra\u00eene une diminution de leur qualit\u00e9 de vie, une utilisation accrue des services de sant\u00e9 et une charge \u00e9conomique importante*.<\/p>\n

Developed by EAPOC, the eAMS is a digital health solution that enhances asthma care by integrating a patient portal and clinical decision support system into existing primary care and pharmacy electronic medical record systems (EMRs). Patients complete an online or mobile questionnaire about their symptoms and medications, which is analyzed by computerized algorithms to generate personalized recommendations based on the latest medical guidelines.<\/p>\n

The system auto-generates asthma control level, medication adjustment recommendations, a personalized self-management asthma action plan, and a prompt to refer to specialty care in cases of severe asthma, and presents this to the provider in their existing workflow. Once approved, the action plan is uploaded to the portal, ensuring that patients have continuous access to their tailored asthma management plan.<\/p>\n

Through its partnership with the CAN Health Network, Unity Health<\/a> Toronto\u2019s St. Michael’s Hospital Academic Family Health Team (one of the largest family health teams in Ontario) has become the latest site to offer EAPOC\u2019s eAMS technology to its primary care patients and providers. This initiative aims to empower both patients and providers with the latest evidence-based guidance, in order to bridge prevalent gaps in asthma care and thereby improve patient outcomes.<\/p>\n
