Au Canada, les patients sont constamment transf\u00e9r\u00e9s des h\u00f4pitaux aux maisons de sant\u00e9 de longue dur\u00e9e et des maisons de sant\u00e9 de longue dur\u00e9e aux h\u00f4pitaux. Bien qu\u2019il doive s\u2019agir d\u2019une transition sans heurts, ce processus peut \u00eatre assez inefficace pour le patient et le fournisseur de soins de sant\u00e9. Un probl\u00e8me courant est que les patients sortent souvent des h\u00f4pitaux avec des documents imprim\u00e9s, notamment une liste de m\u00e9dicaments. En collaboration avec St. Joseph\u2019s de Hamilton, PointClickCare vise \u00e0 limiter ces inefficacit\u00e9s et \u00e0 am\u00e9liorer le partage de renseignements entre les h\u00f4pitaux et les maisons de sant\u00e9 de longue dur\u00e9e.
