Les personnes qui r\u00e9sident dans des \u00e9tablissements de sant\u00e9 de longue dur\u00e9e (SLD) pr\u00e9sentent souvent des troubles cognitifs et de multiples maladies concomitantes, ce qui les expose \u00e0 un risque \u00e9lev\u00e9 de morbidit\u00e9 et de mortalit\u00e9 associ\u00e9 \u00e0 la COVID-19. Le transfert \u00e0 l\u2019h\u00f4pital de ces personnes \u00e2g\u00e9es sensibles pendant la pand\u00e9mie pour des conditions non li\u00e9es \u00e0 la COVID pr\u00e9sentait \u00e9galement un risque suppl\u00e9mentaire pour les r\u00e9sidents \u00e2g\u00e9s. <\/p>\n\n\n\n

Maple est une plateforme de soins virtuels qui permet aux patients d\u2019avoir acc\u00e8s \u00e0 des m\u00e9decins et \u00e0 d\u2019autres fournisseurs de soins de sant\u00e9 de mani\u00e8re rapide et pratique. Cette plateforme a permis aux m\u00e9decins de Trillium Health Partners (THP) d\u2019\u00e9valuer virtuellement les patients pour la COVID-19 (couvert par l\u2019OHIP), de sorte que les patients ne doivent se rendre \u00e0 des visites en personne que lorsque cela est n\u00e9cessaire. Par cons\u00e9quent, les visites des patients sont d\u00e9tourn\u00e9es vers le service des urgences, ce qui permet d\u2019augmenter la capacit\u00e9 de pointe tout en r\u00e9duisant l\u2019exposition des patients aux infections potentielles. <\/p>\n<\/div>\n\n\n\n
