Trillium Health Partners (THP) a constat\u00e9 que les patients sont parfois confus, d\u00e9pass\u00e9s et incertains quant au processus chirurgical, de la phase pr\u00e9-op\u00e9ratoire \u00e0 la phase post-op\u00e9ratoire, ce qui entra\u00eene souvent un sentiment d\u2019anxi\u00e9t\u00e9 tout au long de leur parcours chirurgical. En partenariat avec SeamlessMD, THP a am\u00e9lior\u00e9 les r\u00e9sultats et l\u2019exp\u00e9rience des patients qui se pr\u00e9parent \u00e0 une chirurgie, la subissent et s\u2019en remettent. Ensemble, THP et SeamlessMD ont con\u00e7u un portail \u00e9ducatif virtuel multilingue pour les patients et les soignants. Ce portail offre un acc\u00e8s accru aux renseignements qui permettent aux patients d\u2019acqu\u00e9rir des connaissances \u00e0 toutes les \u00e9tapes de leur parcours chirurgical.<\/p>\n\n\n\n

SeamlessMD est une plateforme tout-en-un pour l\u2019engagement des patients, la surveillance \u00e0 distance et les soins virtuels. Gr\u00e2ce \u00e0 un t\u00e9l\u00e9phone intelligent une tablette ou un ordinateur, il guide virtuellement les patients avant, pendant et apr\u00e8s l\u2019hospitalisation. Il fournit aux patients des plans de soins interactifs comprenant des rappels, des listes de choses \u00e0 faire, des vid\u00e9os et des outils \u00e9ducatifs fond\u00e9s sur des donn\u00e9es probantes. La plateforme utilise \u00e9galement des alertes et des tableaux de bord pour surveiller les patients, ce qui permet au personnel m\u00e9dical professionnel d\u2019intervenir plus t\u00f4t pour les patients \u00e0 risque. <\/p>\n<\/div>\n\n\n\n
