January 22, 2025 (Oakville, ON) – The CAN Health Network is pleased to partner with Careteam Technologies, a Vancouver-based company simplifying the health care journey for patients, caregivers and care providers through an innovative digital health integrated care platform.

The health care system can feel overwhelming for patients, especially those with complex chronic conditions, involving numerous appointments, providers, and follow-up steps to manage. Navigating this fragmented system is challenging, underscoring the need for coordinated provider efforts. Seamless coordination, however, is difficult to achieve without digital tools that integrate all aspects of the patient journey.

Established in 2017, Careteam is a secure, cloud-based, AI-enabled platform that keeps patients and health care teams aligned. It facilitates data sharing across organizations, AI-predictive analytics, and workflow automation, enabling seamless collaboration among providers, patients, and families. This improves quality of care, safety, outcomes, and satisfaction while reducing costs and enhancing capacity and efficiency for clinical teams.

With Careteam, providers can monitor care plans remotely and intervene proactively, empowering patients to take control of their health with support from caregivers and community services. By enhancing care coordination and ensuring timely interventions, it can help ease the burden on emergency departments (ED) and support lower re-hospitalization rates.

To date, the platform has delivered over 20,000 personalized care plans, enabling patients with complex chronic conditions to take timely, informed steps.

Last summer, Careteam was recognized by Fast Company’s 2024 Innovation by Design Awards in the User Experience category for “Humanizing Digital Healthcare” with its exceptional ease of use on any device.

Through this new partnership with the CAN Health Network, Careteam is taking part in a commercialization project led by Bruyère Health that will explore its impact on the Bruyère@Home program. By implementing Careteam’s technology, Bruyère Health is aiming to improve the care outcomes and experiences of patients who are transitioning from hospital to home, while also reducing costs for the healthcare system.

This commercialization project led by Bruyère Health is part of a larger initiative that includes two other large-scale “@Home” programs in Ontario, engaging hospital and community providers, and local Ontario Health Teams. The goal of the project is to determine the digital health approach that will enhance communication and collaboration across health care services, clients, and their care partners, which is particularly important during transitions in care.
