In Canada, patients are constantly transferring from hospitals to long-term care homes and from long-term care homes to hospitals. Although it should be a seamless transition, this process can be quite inefficient for both the patient and health care provider. One common problem is how patients are often discharged from hospitals with printed documentation, including a medication list. These documents get lost in transition leading to increased risk of medication errors and delays in administration of first dose. Additionally, other information, such as the status of co-morbidities or family wishes, is also not available to clinical staff making it more difficult to quickly and accurately diagnosis and treat.
