In Canada, patients are constantly transferring from hospitals to long-term care homes and from long-term care homes to hospitals. Although it should be a seamless transition, this process can be quite inefficient for both the patient and health care provider. One common problem is how patients are often discharged from hospitals with printed documentation, including a medication list. These documents get lost in transition leading to increased risk of medication errors and delays in administration of first dose. Additionally, other information, such as the status of co-morbidities or family wishes, is also not available to clinical staff making it more difficult to quickly and accurately diagnosis and treat.

PointClickCare is the leading cloud-based health care software provider to North America’s long-term post-acute care (LTPAC) and senior care industry. Working with St. Joseph’s in Hamilton, PointClickCare aims to limit these inefficiencies and improve the information sharing between hospitals and long-term care homes. The software eliminates data silos and boundaries between care settings to connect stakeholders to the meaningful insights that help them make better decisions for their businesses, patients and workers.
